Nurse Case Manager
Sovereign Healthcare recognizes that when a loved one is hospitalized or may be
in need of a more acute or “skilled level of care,” families are often at a loss in regard
to knowing what resources are available to them. Because of this, we have added to our staff
a Registered Nurse who in collaboration with the Medical Professionals is proficient in determining
at what level of care our resident may need and what resources need to be mobilized to meet that need.
Sometimes a Home Health Care consult is warranted, and at other times, a short stay in one of our
Care Centers for rehabilitation or other services is what is ordered.
In addition to assisting families in navigating the health care system, our Case Manager is in contact
with the Family, the Hospital Social Worker, when necessary the M.D., and the Wellness Center Nurses
in the Sovereign Healthcare Retirement Communities. We have found that communication across the Care
Continuum enhances the resident’s chances for a successful recovery or at attaining their highest level
of independence. We believe that our residents deserve every consideration in achieving an optimal
wellness condition.
